THIS AMENDMENT No. 1 to Agreement No. a14-181, dated October 27, 2015, is entered into in the State of California by and between the City of Chino Hills, a California general law municipal corporation, hereinafter called "City", and Glenn Lukos Associates, Inc. hereinafter called "Consultant". Amend Agreement A14-181 as follows: 1. Add the attached Scope of Work— Change Order 1 exhibit to the existing Exhibit A 2. Add the attached Direct Expenses exhibit to the existing Exhibit B Original Agreement: $ 40,000 Amendment No. 1 $ 29.320 Total Revised Contract Amount (not to exceed): $ 69,320 All other provisions of the agreement remain in effect. Hensley, City Attorney SCOPE OF WORK CHANGE ORDER 1: ADDITIONAL BIOLOGICAL/REGULATORY SUPPORT FOR PHASE III OF THE PEYTON DRIVE, EUCALYPTUS AVENUE, ENGLISH CHANNEL PROJECT, CITY OF CHINO HILLS TASK I. AS-NEEDED REGULATORY SERVICES SUPPORT GLA will provide as-needed(up to 150 hours)regulatory assistance to the City in support of compliance with the project's aquatic permits with the U.S. Army Corps of Engineers,Regional Water_Quality Control Board, and California Department of Fish and Wildlife. The,as-pecded services include but are not limited to the following: o Construction monitoring during jurisdictional resources removal; o Nesting bird surveys; o Coordination with resource agencies; o Sound monitoring; o Coordination with construction contractor(e.g., sound reduction, Best Management Practices); o GIS analysis of permitted acreage; o Attend weekly field meetings(Ihru 11/2015); o Weekly site checks (thru 11/2015); and o Reporting(monthly, 1 annual)to the resource agencies. DIRECT EXPENSES GLA's direct expenses shall be those costs incurred directly for the CLIENT's project, including, but not limited to, necessary transportation costs including mileage by automobile at the current rate allowed by IRS, Reimbursement for these expenses shall be on the basis of actual charges when furnished by GLA.
