COUNTY and LANDLORD agree that the lease agreement, Contract No. 09-71 is extended from April 1, 2015 to December 31, 2018 with annual rental payments of$1.00. 2. CHANGE those portions of PREMISES and PREMISES LEASED, which now read: 2,500 square feet of offices To read: 2,500 square feet of office and 840 square feet of patio area 3. County per Section 14, ALTERATIONS, shall submit for City approval anticipated improvements to include a seven (7') foot Iron Fence and seven (7') foot block wall to enclose the new patio area along with additional concrete for patio area, a 3' foot high pony wall with gate to run from reception desk to side wall (approximately 10' feet) to block access to the interior office. County will obtain all necessary permits from the City of Chino Hills. 4. COUNTY and LANDLORD agree to add and incorporate into this Lease as Exhibit "E" Chino Hills Wi-Fi Policy as attached. 5. All other provisions and terms of the lease agreement contract No. 09-71 shall remain the same and are hereby incorporated by reference. Date Date Date Revised 1/8/13 Page 3 of 3 EXHIBIT "E" Chino Hills Wi-Fi Policy for Perpetual and Guest Access Consistent with current City policy, the City can provide perpetual access to County Staff Members, however, requires that the specified userid's/ passwords remain confidential and are not to be distributed. Perpetual access will require an email to helodesk@chinohills.ors specifying the staff member's name and job function. The City of Chino Hills will also provide guest access to the County Supervisor's Office as needed. The request will need to be made to the Chino Hills IT Help Desk by either contacting us at 364-4357 (HELP) or by sending an email to helpdesk@chinohills.org. Guest access will be provided upon request. We ask that you provide City IT staff with 4 hours of lead time during normal business hours, so we can facilitate these requests. The IT Division will grant guest access based on the following information; 1. User Name 2. Organization 3, Dates/Times required for Wi-H access IT requires that Windows based machines accessing the Wi-Fi network are protected with antivirus and anti-malware software which prevents the proliferation of virus and internet attacks. All Wi-Fi account access will automatically comply with the City's predetermined web filtering standards, which blocks access to inappropriate and malicious websites.
